The purpose of this study was to develop a Physician's Spiritual Well-Being Scale (PSpWBS). The significance of a physician's spiritual well-being was explored through in-depth interviews with and qualitative data collection from focus groups. Based on the results of qualitative analysis and related literature, the PSpWBS consisting of 25 questions was established. Reliability and validity tests were performed on 177 subjects. Four domains of the PSpWBS were devised: physician's characteristics; medical practice challenges; response to changes; and overall well-being. The explainable (...) total variance was 65.65%. Cronbach α was 0.864 when the internal consistency of the whole scale was calculated. Factor analysis showed that the internal consistency Cronbach α value for each factor was between 0.625 and 0.794 and the split-half reliability was 0.865. The scale has satisfactory reliability and validity and could serve as the basis for assessment of the spiritual well-being of a physician. (shrink)

ABSTRACTDespite interest in exercise as a treatment for anxiety disorders the mechanism behind the anxiolytic effects of exercise is unclear. Two observations motivate the present work. First, engagement of attention control during increased working memory load can decrease anxiety. Second, exercise can improve attention control. Therefore, exercise could boost the anxiolytic effects of increased WM load via its strengthening of attention control. Anxiety was induced by threat of shock and was quantified with anxiety-potentiated startle. Thirty-five healthy volunteers participated in two (...) types of activity, exercise and control-activity. Objectives: To investigate the current situation of completing the informed consent for do-not-resuscitate (DNR) orders among the competent patients with terminal illness and the ethical dilemmas related to it. Participants: This study enrolled 152 competent patients with terminal cancer, who were involved in the initial consultations for hospice care. Analysis: Comparisons of means, analyses of variance, Student’s t test, χ2 test and multiple logistic regression models. Results: After the consultations, 117 (77.0%) of the 152 patients provided informed consent for hospice (...) care and DNR orders. These included 21 patients (17.9%) who signed the consent by themselves, and 96 (82.1%) whose consent sheet was signed only by family members. The reasons why patients were not involved in the discussions toward the consent (n = 82) included poor physical or psychological condition (44.9%), concerns of the consultant hospice team (37.2%), and the family’s refusal (28.2%). On a multivariate analysis, patients’ awareness of their poor prognosis (odds ratio = 4.07, 95% confidence interval = 2.05 to 8.07) and their understanding of hospice care (2.27, 1.33 to 3.89) were two independent factors (p<0.01) that influenced their participation in the discussions or their personal signature in the informed consent. Conclusion: The family-oriented culture in Asian countries may violate the principles of the Patient Self-Determination Act and the requirements of the Hospice Care Law in Taiwan, which inevitably poses an ethical dilemma. Earlier truth-telling and continuing education of the public by hospice care workers will be helpful in solving such ethical dilemmas. (shrink)

Over the past two decades, Victor and Cullen's (Adm Sci Q 33:101-125, 1988) typology of ethical climates has been employed by many academics in research on issues of ethical climates. However, little is known about how managerial practices such as communication and empowerment influence ethical climates, especially from a functional perspective. The current study used a survey of employees from Taiwan's top 100 patent-owning companies to examine how communication and empowerment affect organizational ethical climates. The results confirm the relationship between (...) these two managerial practices and organizational ethical climates. The US neutrino community gathered at the Workshop on the Intermediate Neutrino Program at Brookhaven National Laboratory February 4-6, 2015 to explore opportunities in neutrino physics over the next five to ten years. Scientists from particle, astroparticle and nuclear physics participated in the workshop. The workshop examined promising opportunities for neutrino physics in the intermediate term, including possible new small to mid-scale experiments, US contributions to large experiments, upgrades to existing experiments, R&D plans and theory. The workshop was organized into (...) two sets of parallel working group sessions, divided by physics topics and technology. Physics working groups covered topics on Sterile Neutrinos, Neutrino Mixing, Neutrino Interactions, Neutrino Properties and Astrophysical Neutrinos. Technology sessions were organized into Theory, Short-Baseline Accelerator Neutrinos, Reactor Neutrinos, Detector R&D and Source, Cyclotron and Meson Decay at Rest sessions.This report summarizes discussion and conclusions from the workshop. The structure of Au layers deposited by sputtering on oxidized p-type Si substrates is investigated by a combination of scanning electron microscopy and scanning probe microscopy. The effect of the temperature on the grain structure of the layers has been determined, revealing that an annealing temperature of 300° C results in a larger grain size and smoother surfaces but generates some cracks in the film surface. At an annealing temperature of 500° C, further grain growth is observed, but a high (...) density of cracks and voids also results while there is little enhancement regarding the smoothness of the grain surfaces. The apolipoprotein E gene plays an important role in the pathogenesis of Alzheimer's disease , and amyloid plaque comprised mostly of the amyloid-beta peptide ) is one of the major hallmarks of AD. However, the relationship between these two important molecules is poorly understood. We examined how A treatment affects APOE expression in cultured cells and tested the role of the transcription factor NF-B in APOE gene regulation. To delineate NF-B's role, we have characterized a 1098 nucleotide segment containing the (...) 5'-flanking region of the human APOE gene . Sequence analysis of this region suggests the presence of two potential NF-B elements. To demonstrate promoter activity, the region was cloned upstream of a promoterless luciferase gene. This segment was able to drive expression of luciferase in transient transfections of human fetal glial cells. Promoter activity was stimulated twofold by A treatment. Pretreatment with double-stranded DNA decoy oligonucleotides against NF-B reduced A stimulation. Deletion and mutagenetic analyses demonstrated that the distal NF-B element was functional and showed a strong DNA-protein complex band in gel shift analysis, similar to that from control NF-B consensus element. An anti-inflammatory and anti-NF-B drug, sodium salicylate, significantly blocked A-induced APOE promoter function. Our data provide evidence that upregulation of APOE by A in astroglial cells is mediated by an NF-B-element present in the 5'-flanking region of the APOE gene. (shrink)

Recently implemented Chinese health insurance schemes have failed to achieve a Chinese health care system that is family-oriented, family-based, family-friendly, or even financially sustainable. With this diagnosis in hand, the authors argue that a financially and morally sustainable Chinese health care system should have as its core family health savings accounts supplemented by appropriate health insurance plans. This essay’s arguments are set in the context of Confucian moral commitments that still shape the background culture of contemporary China.

We claim that, as it stands, the Deutsch–Wallace–Everett approach to quantum theory is conceptually incoherent. This charge is based upon the approach’s reliance upon decoherence arguments that conflict with its own fundamental precepts regarding probabilistic reasoning in two respects. This conceptual conflict obtains even if the decoherence arguments deployed are aimed merely towards the establishment of certain ‘emergent’ or ‘robust’ structures within the wave function: To be relevant to physical science notions such as robustness must be empirically grounded, and, on (...) our analysis, this grounding can only plausibly be done in precisely the probabilistic terms that lead to conceptual conflict. Thus, the incoherence problems presented necessitate either the provision of a new, non-probabilistic empirical grounding for the notions of robustness and emergence in the context of decoherence, or the abandonment of the Deutsch–Wallace–Everett programme for quantum theory. We introduce ‘model migration’ as a species of cross-disciplinary knowledge transfer whereby the representational function of a model is radically changed to allow application to a new disciplinary context. Controversies and confusions that often derive from this phenomenon will be illustrated in the context of econophysics and phylogeographic linguistics. Migration can be usefully contrasted with concept of ‘imperialism’, that has been influentially discussed in the context of geographical economics. In particular, imperialism, unlike migration, relies upon extension of the original model (...) via an expansion of the domain of phenomena it is taken to adequately described. The success of imperialism thus requires expansion of the justificatory sanctioning of the original idealising assumptions to a new disciplinary context. Contrastingly, successful migration involves the radical representational re-interpretation of the original model, rather than its extension. Migration thus requires ‘re-sanctioning’ of new ‘counterpart idealisations’ to allow application to an entirely different class of phenomena. Whereas legitimate scientific imperialism should be based on the pursuit of some form of ontological unification, no such requirement is need to legitimate the practice of model migration. The distinction between migration and imperialism will thus be shown to have significant normative as well as descriptive value. (shrink)

To demarcate the limits of experimental knowledge, we probe the limits of what might be called an experiment. By appeal to examples of scientific practice from astrophysics and analogue gravity, we demonstrate that the reliability of knowledge regarding certain phenomena gained from an experiment is not circumscribed by the manipulability or accessibility of the target phenomena. Rather, the limits of experimental knowledge are set by the extent to which strategies for what we call ‘inductive triangulation’ are available: that is, the (...) validation of the mode of inductive reasoning involved in the source-target inference via appeal to one or more distinct and independent modes of inductive reasoning. When such strategies are able to partially mitigate reasonable doubt, we can take a theory regarding the phenomena to be well supported by experiment. When such strategies are able to fully mitigate reasonable doubt, we can take a theory regarding the phenomena to be established by experiment. There are good reasons to expect the next generation of analogue experiments to provide genuine knowledge of unmanipulable and inaccessible phenomena such that the relevant theories can be understood as well supported. In the article the possibility of using a smartphone with an integrated set of necessary technical means for implementing the technology of video recording of vehicle speed is investigated and established. The main algorithmic and software components of a smartphone are proposed, which allow to identify and to accompany a vehicle, as well as to determine its speed. Installation and initial setup of the device is simple. It is only necessary to measure and enter in the device height of its (...) arrangement. In the article the possibility of using a smartphone with an integrated set of necessary technical means for implementing the technology of video recording of vehicle speed is investigated and established. The smartphone has a low cost and power consumption, the ability to operate in the required temperature range, as well as a set of physically built-in necessary technical means: several video cameras, accelerometers, gyroscope, magnetometer, GPS sensor, GSM and Wi-Fi wireless communication. Due to its low power consumption, it can also run on solar panels during the day when no other power source is available. The cost of a smartphone with very good characteristics does not exceed 10,000 UAH. The paper proposes the main components of the algorithmic support of the device. The use of the Lucas-Kanade method for the implementation of tracking and the calculation in the interpixel space of the coordinates of the points of the vehicle's trajectory will improve the accuracy of measurements of the traveled distance and the vehicle's speed on short sections of the trajectory. The proposed algorithm for calculating the vehicle speed requires entering only one parameter - the height of the device. Therefore, installation and initial setup of the device is simple. It is enough to measure and enter this parameter into the device. Large-scale introduction of a device for measuring vehicle speed, built on the basis of a smartphone, is possible due to relatively small investments in the development of its mechanical part, algorithmic and software. The subjective Everettian approach to quantum mechanics presented by Deutsch and Wallace fails to constitute an empirically viable theory of quantum phenomena. The decision theoretic implementation of the Born rule realized in this approach provides no basis for rejecting Everettian quantum mechanics in the face of empirical data that contradicts the Born rule. The approach of Greaves and Myrvold, which provides a subjective implementation of the Born rule as well but derives it from empirical data rather than decision theoretic arguments, (...) avoids the problem faced by Deutsch and Wallace and is empirically viable. However, there is good reason to cast doubts on its scientific value. (shrink)

In this article, I examine Luhmann’s, Bunge’s and others’ views on emergence, and argue that Luhmann’s epistemological construal of emergence in terms of Totalausschluss (total exclusion) is both ontologically flawed and detrimental to an appropriate understanding of the distinctive features of social emergence. By contrast, Bunge’s rational emergentism, his CESM model, and Wimsatt’s characterization of emergence as nonaggregativity provide a useful framework to investigate emergence. While researchers in the field of social theory and sociology tend to regard Luhmann as the (...) sole representative of systems theory, the latter has been characterized by its diversity, and the writings of such systems theorists as Mario Bunge deserve more (critical) attention from social researchers than they receive at present. Finally, this article suggests that the perennial debate over methodological individualism and holism in social science may make real progress if such ambiguous terms as reduction and reductionism are elucidated before they are employed. Background: Although ethics consultation has been introduced to clinical practice for many years, the results of empirical studies to evaluate the effectiveness of ethics consultation are still controversial. The design of randomised controlled trials is considered the best research design to evaluate the effect of a clinical practice on the outcomes of interests. In order to understand the effects of ethics consultation, we conducted this search for studies with the design of randomised controlled trials to evaluate ethics consultation.Objective: To provide (...) an integrated review of studies with the design of randomised controlled trials to evaluate the effectiveness of ethics consultation.Methods: PubMed was used to search for studies using the randomised controlled trial design to evaluate the effectiveness of ethics consultation. The search term used was “ethics consultation”. The selection criterion was limited to “randomised controlled trial”.Results: Four articles that met both search criteria were retrieved. One of these articles reported a study that did not actually use the design of a randomised controlled trial and is excluded from the following discussion.Conclusions: To apply randomised controlled trials to evaluate the effectiveness of ethics consultation is extremely difficult as long as two issues are not resolved: the standardisation of ethics consultation and a placebo for ethics consultation to eliminate the placebo effect. Thus, the results generated by the design of randomised controlled trials are always problematic. Furthermore, as long as the two issues exist, the results generated by the design of quantitative research methods always pose problems. (shrink)

On one popular view, the general covariance of gravity implies that change is relational in a strong sense, such that all it is for a physical degree of freedom to change is for it to vary with regard to a second physical degree of freedom. At a quantum level, this view of change as relative variation leads to a fundamentally timeless formalism for quantum gravity. Here, we will show how one may avoid this acute ‘problem of time’. Under our view, (...) duration is still regarded as relative, but temporal succession is taken to be absolute. This article attempts to assess Mario Bunge’s important but widely neglected criticisms of dialectics. It begins by providing a contextualized interpretation of Friedrich Engels’s metaphysics of the dialectics of nature before embarking on a detailed discussion of Leon Trotsky’s and contemporary “dialectical” scientists’ views on materialist dialectics. It argues that while some of Bunge’s criticisms are eminently sensible, the principles underlying the works of dialectical scientists are compatible with Bunge’s emergentist and systemic approach and can shed light on such issues (...) as the levels of organization, the diachronic and synchronic aspects of emergence, and the individualism-holism-systemism trilemma. This article also submits that dialectics is best interpreted as a guideline for a philosophy of change instead of a magical wand that liberates the investigator from study of facts. Understood as something that serves heuristic purposes, dialectics can be sensibly utilized by scientists to shore up or refine their methodological principles and thereby to facilitate empirical research. The `problem of time' is a cluster of interpretational and formal issues in the foundations of general relativity relating to both the representation of time in the classical canonical formalism, and to the quantization of the theory. The purpose of this short chapter is to provide an accessible introduction to the problem.

Starting from a generalized Hamilton-Jacobi formalism, we develop a new framework for constructing observables and their evolution in theories invariant under global time reparametrizations. Our proposal relaxes the usual Dirac prescription for the observables of a totally constrained system and allows one to recover the influential partial and complete observables approach in a particular limit. Difficulties such as the non-unitary evolution of the complete observables in terms of certain partial observables are explained as a breakdown of this limit. Identification of (...) our observables relies upon a physical distinction between gauge symmetries that exist at the level of histories and states, and those that exist at the level of histories and not states. This distinction resolves a tension in the literature concerning the physical interpretation of the partial observables and allows for a richer class of observables in the quantum theory. There is the potential for the application of our proposal to the quantization of gravity when understood in terms of the Shape Dynamics formalism. (shrink)

Symplectic reduction is a formal process through which degeneracy within the mathematical representations of physical systems displaying gauge symmetry can be controlled via the construction of a reduced phase space. Typically such reduced spaces provide us with a formalism for representing both instantaneous states and evolution uniquely and for this reason can be justifiably afforded the status of fun- damental dynamical arena - the otiose structure having been eliminated from the original phase space. Essential to the application of symplectic reduction (...) is the precept that the first class constraints are the relevant gauge generators. This prescription becomes highly problematic for reparameterization invariant theories within which the Hamiltonian itself is a constraint; not least because it would seem to render prima facie distinct stages of a history physically identical and observable functions changeless. Here we will consider this problem of time within non-relativistic me- chanical theory with a view to both more fully understanding the temporal struc- ture of these timeless theories and better appreciating the corresponding issues in relativistic mechanics. For the case of nonrelativistic reparameterization invariant theory application of symplectic reduction will be demonstrated to be both unnec- essary; since the degeneracy involved is benign; and inappropriate; since it leads to a trivial theory. With this anti-reductive position established we will then examine two rival methodologies for consistently representing change and observable func- tions within the original phase space before evaluating the relevant philosophical implications. We will conclude with a preview of the case against symplectic re- duction being applied to canonical general relativity. (shrink)

In 1981 Unruh proposed that fluid mechanical experiments could be used to probe key aspects of the quantum phenomenology of black holes. In particular, he claimed that an analogue to Hawking radiation could be created within a fluid mechanical `dumb hole', with the event horizon replaced by a sonic horizon. Since then an entire sub-field of `analogue gravity' has been created. In 2016 Steinhauer reported the experimental observation of quantum Hawking radiation and its entanglement in a Bose-Einstein condensate analogue black (...) hole. What can we learn from such analogue experiments? In particular, in what sense can they provide evidence of novel phenomena such as black hole Hawking radiation? (shrink)

A physically consistent semi-classical treatment of black holes requires universality arguments to deal with the ‘trans-Planckian’ problem where quantum spacetime effects appear to be amplified such that they undermine the entire semi-classical modelling framework. We evaluate three families of such arguments in comparison with Wilsonian renormalization group universality arguments found in the context of condensed matter physics. Our analysis is framed by the crucial distinction between robustness and universality. Particular emphasis is placed on the quality whereby the various arguments are (...) underpinned by ‘integrated’ notions of robustness and universality. Whereas the principal strength of Wilsonian universality arguments can be understood in terms of the presence of such integration, the principal weakness of all three universality arguments for Hawking radiation is its absence. The analysis of the temporal structure of canonical general relativity and the connected interpretational questions with regard to the role of time within the theory both rest upon the need to respect the fundamentally dual role of the Hamiltonian constraints found within the formalism. Any consistent philosophical approach towards the theory must pay dues to the role of these constraints in both generating dynamics, in the context of phase space, and generating unphysical symmetry transformations, in the context of a hypersurface (...) embedded within a solution. A first denial of time in the terms of a position of reductive temporal relationalism can be shown to be troubled by failure on the first count, and a second denial in the terms of Machian temporal relationalism can be found to be hampered by failure on the second. A third denial of time, consistent with both of the Hamiltonian constraints roles, is constituted by the implementation of a scheme for constructing observables in terms of correlations and leads to a radical Parmenidean timelessness. The motivation for and implications of each of these three denials are investigated. (shrink)

Cheating is a serious issue among business students worldwide. However, research investigating the social factors that may help prevent cheating in Chinese higher education is rare. The present study examined two key social relationship factors of perceived teacher-student relationships and peer relationships by the students. It attempted to build a model which addressed the effects of two variables on Chinese business students’ cheating behaviour: the teacher’s approachability and the relationship goal of the students. Two important social influence factors were also (...) tested as mediators: neutralizing attitudes and perceived cheating norms of the students. A student survey was conducted with 1329 questionnaires collected. The results showed the negative effects of both social relationship variables on cheating, and that their effects were fully mediated by neutralizing attitudes. Moreover, perceived cheating norms fully mediated the effect of the teacher’s approachability, but not so for the relationship goal of the students. This study provided novel insights and recommendations for promoting academic integrity in Chinese business schools and universities. (shrink)

We present a Bayesian analysis of the epistemology of analogue experiments with particular reference to Hawking radiation. Provided such experiments can be externally validated via universality arguments, we prove that they are confirmatory in Bayesian terms. We then provide a formal model for the scaling behaviour of the confirmation measure for multiple distinct realisations of the analogue system and isolate a generic saturation feature. Finally, we demonstrate that different potential analogue realisations could provide different levels of confirmation. Our results thus (...) provide a basis both to formalise the epistemic value of analogue experiments that have been conducted and to advise scientists as to the respective epistemic value of future analogue experiments. Hamiltonian constraints feature in the canonical formulation of general relativity. Unlike typical constraints they cannot be associated with a reduction procedure leading to a non-trivial reduced phase space and this means the physical interpretation of their quantum analogues is ambiguous. In particular, can we assume that “quantisation commutes with reduction” and treat the promotion of these constraints to operators annihilating the wave function, according to a Dirac type procedure, as leading to a Hilbert space equivalent to that reached by quantisation (...) of the problematic reduced space? If not, how should we interpret Hamiltonian constraints quantum mechanically? And on what basis do we assert that quantisation and reduction commute anyway? These questions will be refined and explored in the context of modern approaches to the quantisation of canonical general relativity. (shrink)

A novel approach to quantization is shown to allow for superpositions of the cosmological constant in isotropic and homogeneous mini-superspace models. Generic solutions featuring such superpositions display: i) a unitary evolution equation; ii) singularity resolution; iii) a cosmic bounce. Explicit cosmological solutions are constructed. These exhibit characteristic bounce features including a ‘super-inflation’ regime with universal phenomenology that can naturally be made to be insensitive to Planck-scale physics.
